What is Person-Centred Care?

At Bluebird Care we adopt a person-centred care approach to homecare and independent living support. This means that the customer is at the heart of all decisions made about his or her care.

This is a very important ethos as it means that we are directed by the person receiving care. Sometimes this can include involvement from other medical professionals and/or family members.

In this blog post we set out the steps we at Bluebird Care take in providing person-centred care to our customers.

 

Person-centred care planning at Bluebird Care

Step 1

A prospective customer or a member of their family will contact us and inquire about our ability to offer care in this particular circumstance. Sometimes we will be contacted by a medical professional such as a GP, Public Health Nurses or a Discharge Nurse in a hospital.  They will give an outline of the care requirements of the individual and we agree to set up a meeting with the person and/or their family/medical team.

Step 2

At the first meeting with the prospective customer we get an insight into their care needs. The meeting is attended by a Bluebird Care Care Manager and also a Care Planner. The meeting will centre around looking at the individual’s medical history, family circumstances, reasons for their current care requirements, family and social circle, interests, hobbies, level of independence. Really we want to get a picture of the life they want to lead and how we can support them in achieving that. Homecare support can range from 30-minute visits to more longer term and more intense care. But it is entirely up to the individual and his/her family what they want and need. We take notes of all the relevant information to prepare a person-centred homecare support package.

Step 3

Following the one-to-one meeting the Bluebird Care care team will devise a customised package of care and support relevant to the customers’ needs. This will be very in-depth and will have a schedule of tasks, times, roles and responsibilities of the care team and also reporting mechanisms for the individual/their family. This care plan is presented to the individual and their family for review.

Step 4

When we are assigned a care package with a customer we will then introduce the members of our team who will be working directly with you. They are usually Care Assistants and Personal Assistants. It is very important that a rapport and a bond is established between the Bluebird Care team members and the customer and their family and this first initial meeting will establish this.

Step 5

As with any plan, it needs to be constantly monitored and regularly reviewed. Bluebird Care review care plans monthly to ensure that we take account of changes in the customers’ medical situation, lifestyle, family network and any other relevant factors and the care plan is updated accordingly.

2015 Bluebird Care Franchise Awards

And the winners are…

Each year Bluebird Care Ireland Head Office presents the Franchise of the Year Awards at our end of year network meeting. On Thursday (10th December) our 24 Directors gathered at the Aviva Stadium to plan ahead for 2016.

Managing Director, Brian MacGoey, paid tribute to the network’s 1,500+ plus staff who he says, “are the cornerstone our business. From our Care Managers, to our Supervisors, Nurses, Office Managers and Healthcare Assistants, their commitment to delivering quality care coupled with constant improvement ensures our customers remain at home. Our customers lives are changed because of our staff – whether they are suffering from or are recovering from illness or if they live with a disability and need extra support – Bluebird Care staff are there to make life easier. As a company we look forward to increasing our staff compliment in 2016 due to continued growth.”

The meeting also served as an opportunity to shine a light on our top performing franchises across the country.

Announcing the awards Operations Director, Eddie O’Toole said that 2015 was a particularly good year for the network with significant business wins and aggressive recruitment a feature due to continuing growth.

Franchise of the Year Award

Eddie O’Toole described the winners of this top award as a team who have re-structured their business by introducing new systems and processes and investing heavily in their team. The winners will attend the Interim Healthcare International Conference in Florida in April 2016.

“This Franchise has focused much of the past year on working on their business and it has certainly paid off. They have set the business up for continued expansion and showed grit, determination and commitment in improving structures. 

I am delighted to present the Bluebird Care Franchise of the Year Award to Bluebird Care Sligo.”

Spirit of Bluebird Care Award

The Sprit of Bluebird Care Award is presented to a franchise that has shown deep commitment in the past 12 months to the company brand, network and their own business. Announcing the award winner, Eddie O’Toole described this Director as one who “never stops promoting the brand at every opportunity and connecting with key advocates for homecare in his region.”

He went on: “I’m thrilled to announce that the winner of the Spirit of Bluebird Care Award is Michael Corcoran of Bluebird Care Wexford. Michael has really pushed his brand in the south east region by making strategic connections, constantly working in his community and engaging in conversations in the healthcare sector.”

Marketing Excellence Award

Joanne Sweeney-Burke of Digital Training Institute announced the winners of the third annual marketing excellence award.

“This Bluebird Care award goes to an office that is constantly seeking new ideas and indeed suggesting new ideas. This office has been pro-active in implementing and using the marketing collateral provided throughout the year.

This office has stepped into photo shoots and speaking slots when Eddie wasn’t available and is highly responsive to all marketing communications.

I’m delighted to honour Bluebird Care North East with this year’s Marketing Excellence Award presented to co-Directors Kieran Hallinan and Marty Kelly.”

Launch of Food for the Soul Autumn Schedule

This week chef Gary O’Hanlon helped us launch our 2015 autumn/winter Food for the Soul cookery demo schedule. Gary will take to the road with us as we visit six regional towns as we continue to host our free cookery demos that see Gary cook dishes from our award-winning cookbook, Food For The Soul.

Gary will re-appear on our TV screens this season with a new series of The Restaurant featuring Marco Pierre White as the new resident critic. While Gary was keeping tight-lipped about the stars lined up for this series, he did promise we would be served up some more great dishes, kitchen nightmares along with plenty of drama!

Our food for the soul audiences love to question Gary about The Restaurant, his top cooking tips, his culinary failures and his time in the United States. Gary is always open to plenty of questions so be sure to reserve your free places/s at this season’s cookery demos.
